What eye surgery in China actually costs

Eye care splits into two different price worlds. Cataract surgery has a number China publishes openly: a national bulk purchase cut intraocular lens prices by 60%, and the selected prices are public. Laser vision correction has no official figure anywhere — not in China, not in the US government record — so we handle it differently. Both stories are below, with sources.

The scope problem

One department, two completely different price systems

Almost every English page about “eye surgery in China” blurs two procedures that could not be priced more differently:

Medical

Cataract surgery

A medically necessary procedure, mostly for older patients. The lens implant went through China’s national bulk-purchase system, so its price is published and regulated. Part of the cost is usually covered by China’s basic medical insurance for insured patients.

Elective

Laser vision correction

LASIK, SMILE and ICL are elective procedures paid entirely out of pocket, everywhere in the world. There is no government price list for them in China — and no national statistics body publishing averages either. Prices exist only as quotes.

Why the split matters. For cataract surgery, part of the price is verifiable before you ever contact a hospital. For vision correction, nothing is — which makes the written quote the only real price. The two halves of this page follow that split.

Level A · Official pricing

The lens that got 60% cheaper by regulation

In November 2023, China’s National Healthcare Security Administration ran the fourth national bulk purchase of high-value medical consumables, covering intraocular lenses. 126 companies won slots, including the major multinational manufacturers. The selected prices took effect nationwide from May to June 2024, and the headline figures were published by the agency itself.

Lens category Price before the purchase Selected price after
Monofocal, non-toric — the best-selling domestic model ¥2,500 ≈ ¥800
New-generation monofocal (US manufacturer) ¥10,000 ≈ ¥1,100
Extended depth-of-focus (newly launched model) ¥11,000 ≈ ¥3,500
Trifocal, non-toric — premium category ¥23,000 ≈ ¥8,900
All intraocular lens categories, average — −60%

What this price is. The lens only — the implanted consumable. It does not include the surgeon’s fee, the operating room, anaesthesia, or pre- and post-operative care.

What a patient actually pays. Less than the lens price suggests, for insured patients: for basic monofocal lenses within China’s medical insurance, a large share of the cost is reimbursed at provincial rates, and since 2025 several provinces have folded the selected lens prices fully into the insurance payment scope. The exact out-of-pocket figure depends on your insurance status — it is a per-person answer, not a national one.

Why this row is strong evidence. These are not quotes from a clinic. They are the outcome of a government bidding round, published by the agency that ran it, with named manufacturers and named prices. Very few countries publish lens prices this way.

The other lines of the bill

The surgery fee: regulated, but not nationally priced

The surgical part of a cataract bill — phacoemulsification with lens implantation — is a government-priced medical service item in public hospitals. That means it is not free-form: each province sets a price基准 under the national item-structure guideline, and public hospitals charge within that framework, with premium lenses and laser-assisted techniques billed as clearly separated add-ons.

What China does not publish is a single national figure for the whole procedure — and the honest reason is that the fee is set province by province, so a national average would be a made-up number. This is also why we do not print the “¥5,000–30,000” style ranges that circulate on health-content sites: they aggregate marketing pages, not price schedules.

The practical move. Ask a hospital for a written breakdown with three lines: surgery fee, lens price and model, and what your insurance status changes. A hospital that answers those three lines in writing is giving you a comparable quote; one that answers with a single bundled number is not.

Level C · No published figure

Laser vision correction: the price only exists as a quote

We looked for a sourced national price for SMILE, LASIK or ICL surgery in China — from a government body, a hospital price-publicity page, or an industry association. There isn’t one. The wide ranges that fill Chinese health-content sites and English-language “medical tourism” pages are aggregations of clinic marketing, sometimes attributed to individual doctors answering questions on consumer platforms. None of it traces to a document. So this section contains no number — and that is a decision, not an omission.

What we can give you is the anatomy of the bill, which is stable across hospitals:

术前检查Pre-operative work-up

Corneal topography, pachymetry, refraction, dry-eye assessment, dilated fundus exam — typically 20-odd tests over half a day. Some hospitals deduct this fee from the surgery price if you proceed; ask.

手术费The surgery itself

Driven by the platform: surface ablation, femtosecond-assisted LASIK, SMILE with its single-purpose laser, or ICL with a per-lens implanted device. The technology line explains most of the spread between a hospital’s cheapest and dearest quote.

术后用药与复查Medication and follow-ups

Anti-inflammatory and lubricant drops for weeks, plus scheduled reviews. Confirm how many follow-up visits are included and what an extra visit costs. Enhancement surgery, if ever needed later, is normally a new bill.

书面总价The written total

The only figure worth comparing. Ask: “Does this total include the work-up, all medication, and all standard follow-ups? Is enhancement covered?” — and get the answers in writing, in one document.

A warning that costs nothing to follow. Aggressive low-price advertising exists in this market, and it works the same way everywhere: the advertised figure covers the narrowest candidate and the barest scope. Treat any price you did not receive in writing, for your specific prescription, as marketing — not as a quote.

Level B · Industry statistics

The US side: one good number, and a gap

For laser vision correction, the US has something China lacks: an industry body that publishes an average. The American Refractive Surgery Council’s website states an average price of $4,492 for both eyes (about $2,250 per eye) for standard LASIK, and explains that the famous $99-per-eye advertisements apply only to minimal prescriptions and exclude pre- and post-operative care. We link the page below. One caveat: it does not state a statistical year, so we describe it as “as displayed on 31 October 2026”, not as a 2026 average.

For cataract surgery, the US regresses to the same gap as everywhere else: no national statistical body publishes an average price, because the bill depends on the facility, the insurer and the lens. The comparison you can honestly make is component-level: the selected price of a premium trifocal lens in China’s national purchase (≈¥8,900, roughly $1,250) is a verifiable figure; ask a US surgery centre what the same lens category is billed at and you will be told a number that depends on your insurance — or no number at all until after the claim.

The asymmetry, stated plainly. For cataract lenses, China publishes the regulated price and the US does not publish a comparable figure. For LASIK, the US publishes an industry average and China does not. Each country is transparent about exactly half of this page — which is itself the most useful thing to know before reading anyone’s comparison table.

Verification

How to check an eye hospital yourself

We list no hospitals here. What we give you is the four checks, in Chinese, that a hospital cannot fake.

医疗机构执业许可证Practice licence

Must be displayed, and its licensed scope (诊疗科目) must include ophthalmology (眼科). A licence for medical aesthetics or optometry alone does not cover intraocular surgery.

医师执业证书Practitioner licence

Ask for the licence of the surgeon who will operate, with the registration scope matching ophthalmic surgery. Refractive surgery carries additional institutional requirements in China; a hospital doing it routinely can state its approval without hesitation.

设备型号The platform, by name

Ask which laser or lens platform the quoted price uses, and have the model written down. “Imported equipment” is not an answer; a model name is.

国际部 / 国际医疗部International department

Larger public hospitals operate one, with English-speaking staff and prices above the standard schedule. Confirm which schedule your quote uses — the difference is real and not always disclosed up front.

Questions we get

Straight answers

Is cataract surgery in China cheaper than in the US?

The lens component is regulated and published: a mainstream monofocal lens now carries a selected price near ¥800 after the national purchase. We do not print a total-procedure comparison, because the US side has no sourced figure and the Chinese surgery fee is set province by province. What we can say is component-level, and it is sourced.

Is LASIK cheaper in China than in the US?

We do not publish a comparison, because the China side has no sourced national figure — only quotes. The honest method: obtain a written total from a Chinese hospital using the four-line anatomy above, and set it against the ARSC average of $4,492 for both eyes on the US side. Two scoped numbers beat one savings percentage built on air.

Will insurance cover any of this?

In China, cataract surgery is within basic medical insurance for insured residents, with rules varying by province and by lens category. Laser vision correction is elective and not covered, anywhere. For US visitors, US insurers generally have no direct network in mainland China; some hospital international departments issue itemised invoices suitable for out-of-network reimbursement claims. Ask before treatment, not after.

How do I choose between SMILE, LASIK and ICL?

By your eyes, not by price — corneal thickness, prescription, pupil size and dry-eye status decide candidacy, and the same work-up that decides it also produces the real quote. This is a clinician question, and we do not answer it for you. What the price page can say: whichever procedure is right for you, get its total in writing with the scope lines above included.

What if something goes wrong after I fly home?

For cataract surgery, post-operative reviews are scheduled within days and weeks — so travelling patients should plan the first review period in China or confirm a home-country ophthalmologist willing to see them. For refractive surgery, ask the hospital in advance what its enhancement and complication policy covers for patients who have left the country, and get that in writing with the quote.
