Understanding China’s Hospital Tiers: What “3A” Actually Means

三级甲等 — “Tier 3, Grade A”, usually shortened to 3A or “tier-3A” — appears in almost every description of a Chinese hospital. It is the top of China’s hospital classification system. What it certifies is specific, and what it does not certify matters just as much.

Two axes, one label

China grades hospitals on two dimensions at once. The tier (级) describes scale and scope: how large the institution is, how many specialties it must staff, what level of care it is licensed to deliver. The grade (等) is a quality-and-management score within that tier. A hospital’s public label — 三级甲等, printed on its licence and its signage — combines both.

Tier What it describes Typical role
Tier 3 (三级) Large regional hospitals with the full specialty spread, research and teaching duties, and the capability for complex procedures. Referral centre; complex and serious cases
Tier 2 (二级) Mid-size county or city hospitals covering the main specialties at a smaller scale. Common inpatient and emergency care
Tier 1 (一级) Community-level institutions and clinics with basic services. Primary care, routine visits

Within each tier, hospitals are graded A, B or C by an evaluation of management, services and clinical standards. Grade A (甲等) is the top grade within the tier. So “tier-3A” means: a large regional hospital, with the full range of specialties, that scored at the top of its evaluation cycle.

The useful shortcut. For a foreign patient deciding where to go, the practical reading is: 三级甲等 = a major hospital built to handle complex cases, evaluated and re-evaluated by the authorities. It is the strongest single institutional signal in the system — and it is checkable, because the label appears on the institution’s licence (医疗机构执业许可证).

What tier-3A does not tell you

  1. It is not a rating of individual doctors. The grade describes the institution. Skill varies widely between departments and between practitioners inside the same 3A building. Always verify the practitioner’s own licence (医师执业证书) and registered scope.
  2. It is not a quality guarantee for a specific procedure. A 3A hospital with a strong cardiology department may have an average dental department. Grade follows the institution, not your treatment.
  3. It does not predict price. Public-hospital prices follow provincial schedules regardless of prestige; premium lenses, specialist fees and international-department rates sit on separate lines.
  4. It is not the same as accreditation. JCI or other international accreditations are separate, voluntary, and mostly held by private and international hospitals. Most excellent Chinese 3A hospitals hold none — absence proves nothing in either direction.

Other words you will meet, decoded

综合医院General hospital

Covers the full specialty spread. A safe default when you do not know which specialty you need.

专科医院Specialty hospital

Dedicated to one field — 口腔医院 (stomatology/dental), 眼科医院 (ophthalmology), 肿瘤医院 (oncology). Often a tier-3A institution within its field, with deep case volumes.

国际部 / 特需医疗International / VIP services

Separate units inside public hospitals with English-speaking staff and shorter waits, priced above the standard schedule. Confirm which schedule a quote uses.

门诊部Clinic / outpatient department

A smaller licensed unit — not a hospital. Common in dental and aesthetics chains. Perfectly legitimate, but it cannot offer what a hospital licence covers, and the label should be priced accordingly.

How to check any of this in five minutes

Ask the institution for, or photograph, its medical institution practice licence (医疗机构执业许可证): the institution class and tier are printed on it. Then ask for the practitioner licence (医师执业证书) of the specific doctor who will treat you, and check the registered scope matches the treatment. Both licences carry registration numbers. An institution that will not show them has answered your question.

The boundary we hold. Tiers and grades describe systems, not outcomes. We publish no hospital rankings and no clinic lists, because verified outcome data does not exist in public form. What the licence system lets you do — and what this guide equips you to do — is verify that an institution is what it claims to be before you send money.
